A shoulder bag hangs from one point, so the load pulls down on one side and many people compensate by lifting that shoulder. The strap does more than the contents

If a bag you love has a thin strap, a wider replacement strap is often available separately, which is worth knowing before deciding the bag itself has to go.

Where it sits changes what carries it

Worn crossbody, the strap runs from shoulder to opposite hip and the load is shared by the torso. Worn on one shoulder, everything hangs from a joint that was not designed to hold it, and the shrug you do to stop it sliding is what tightens the muscle. If a bag can only be worn one way and that way is one shoulder, it is a bag for short distances.

Empty it before you replace it

Worth saying because it is free, and because lightening the load is standard advice rather than a last resort: most bags carry things that have not been needed in months. Before buying a smaller bag, try carrying the current one with only what the smaller one would hold, and see whether it is the bag or the volume.

For weight distribution, yes — both shoulders and the load close to the spine. It is not always better for the occasion, and a backpack worn on one shoulder is a shoulder bag with a worse strap.
Should I swap shoulders?
It spreads the wear, and it does not solve the mechanics. If the ache is daily rather than occasional, moving the strap across the body does more than alternating which side it hangs from.
Does a heavier bag mean a better bag?
Structured leather weighs more than coated canvas before anything goes in it, and that weight is part of what you carry all day. A bag that keeps its shape has real advantages; on a long day, its own weight is one of the costs.
